I, Devi, wife of Mohamed Anvar Sadath, Indian, aged about 39 years, residing at No. 44, Stalin Nagar, Karaikal Town and District Munsifi, do hereby solemnly affirmed and oath as follows:

Whereas, my name has been mentioned as 'Suresh @ Prakash' in my Elector's Photo Identity Card under No. HJN0204016 and the same name (in Tamil) '•' has been entered in my Family Ration Card issued by the Department of Civil Supplies and Consumer Affairs, Puducherry.
I depose that the names 'Suresh', 'Prakash', 'Suresh @ Prakash' and '•' are refer to one and the same person i.e., myself.

Further, I state and declare that due to sentimental as well as astrological reasons, I have desired to and keep my name as 'Marthina' only.
Therefore, I declare that hereafter, I shall be called and known as 'Marthina' only. Thus, I am swearing/executing this affidavit in this regard.
