I state that my name is mentioned as 'P Saminathan' in my Aadhaar Card bearing No. 4797 4047 6258.
I state that my name is mentioned as '' in my Family Ration Card No. 401856 issued by the Department of Civil Supplies and Consumer Affairs, Puducherry.
I state that my name is mentioned as 'P. Saminathan' in my Office Order No. 1916/DDE(W)/Estt./E6/2018/13 issued by the Office of the Deputy Director of Education (Women), Government of Puducherry.
I was a Hindu by birth and my birth name was 'Bharathi' recently, I had embraced Islamic faith. Upon conversion of religion, I had renounced my birth name 'Bharathi' and assumed to myself the Muslim name 'Jasmine Begam (↔)'.
I hereby declare that I shall henceforth be known, called, referred and identified only as 'Jasmine Begam (↔)'.

ADDENDUM
After the existing para 2(x) and the Table thereunder in the G.O. Ms. No. 83, dated 27th November, 2013 of the Home Department, Government of Puducherry, the following shall be added, namely:-
"(xi) The Additional Resident Commissioner-I/ Additional Resident Commissioner-II, P ud u cher r y Go ver n ment Gu est Ho use, New Delhi are designated as Nodal Authorities at New Delhi who shall forward the documents to the Authorised Officer at Puducherry for verification and Authentication/Legalisation. The Authorised Officer, Puducherry shall be the single point of communication for Nodal Officer at New Delhi for Union territory of Puducherry." 2. The addendum shall come into force on and from the date of its publication in the Official Gazette.
