I, S. Jayabalan, son of Sundaram, aged about 55 years and residing at No. 14, Adimoolam Nagar, Velrampet, Puducherry-605 004, do hereby solemnly and sincerely affirm on oath as follows:

I am the deponent herein.

I submit that my name is mentioned as 'Djeabalane' in my Birth Certificate under Registration No. 263, issued by the Villianur Commune Panchayat, Puducherry.

I submit that my name is mentioned as 'S. Djeabalane' in my Transfer Certificate in No. 02711 issued by the Headmaster, Vivekananda Government High School, Villianur, Puducherry-605 110.

I submit that my name is mentioned as 'S Djeabalane' in my Aadhaar Card No. 9019 4338 8722.

I submit that my name is mentioned as 'Jayabalan' in my PAN Card No. AYFPJ0457R issued by the Income Tax Department and in my Voter's Identity Card No. PY/01/008/030777.

I submit that my name is mentioned as 'Jayabalan S' in my Savings Account No. 450646267 of Indian Bank, Mudaliarpet Branch, Puducherry.

I submit that my name is mentioned as '➢↔' in my Family Ration Card No. 118688 issued by the Department of Civil Supplies and Consumer Affairs, Puducherry.

I s u b m i t t h a t m y n a m e i s m e n t i o n e d a s 'S. Jayabalan' in my Office Identity Card issued by the Directorate of Survey and Land Records, Department of Revenue and Disaster Management, Government of Puducherry.

Thus, all the names 'Djeabalane', 'S. Djeabalane', 'S Djeabalane', 'Jayabalan', 'Jayabalan S', '➢↔' and'S. Jayabalan' are refer to one and the same person that is myself only.

H e r e a f t e r, I s h a l l b e k n o wn b y t h e n a me 'S. Jayabalan'.

I hereby declare that I shall at all times, hereafter, in all records, deeds and writings and in all proceedings, dealings and transactions, private as well as upon all occasions whatsoever use and sign the name of 'S. Jayabalan' as my name in place and in substitution of my former names.

I state and declare that I was born on 3-11-1966, but, my birth was not registered before anywhere and named me as 'Devapatcham' and the same is entered into my School Transfer Certificate issued by Headmistress, St. Mary's High School, Karnataka State for the year 1980-1981 under Admission No. 250, Voter 's Identity Card, dated 13-2-2016 under No. BQG0202804, Aadhaar Card, dated 3-12-2015 under No. 4373 0844 2918 and Aadhaar Card of my daughter viz., Priyadharshini, dated 28-12-2015 bearing No. 6430 7435 7028 and in my son viz., Praveen Anbu, dated 29-7-2016 bearing No. 2062 7270 5202.

Further, I state and declare that whereas, my name is also spelled as 'Devapatchan' and the same is entered into the Birth Certificate of my daughter viz., Priyadharshini, dated 5-8-1998 bearing Registration No. M/1998/08871, dated 5-8-1998.

Further, I state and declare that apart from my said birth name, I was also having one nick name as 'Deva' and the name is entered into my son's Birth Certificate viz., Praveen Anbu bearing Registration No. M/1999/14413, dated 8-11-1999 and in my Family Ration Card, dated 5-7-2015 under No. 431923 (in Tamil).

I state that though, I am having the aforesaid names, I am willing and desire to have and possess my name as 'Devapatcham' only.

Therefore, hereafter, I shall be called only as 'Devapatcham' in future.
