I, Vigneswaran, son of Jayakumar, aged 25 years and residing at the house bearing Door No. 19, Muthamizh Street, Shanmuga Nagar, Karuvadikuppam, Puducherry-605 008, do hereby solemnly and sincerely affirm to whomsoever it may concern as follows:
I am the deponent herein. I state that in my Birth Certificate under Registration No. 1307/92 registered with the Municipal Commissioner, Nagapatinam, Tamil Nadu, my name has been mentioned as 'Vigneswaran'.

In my grandfather Dhanesegara Gurukkal's Family Ration Card under No. 147231 issued by the Department of Civil Supplies and Consumer Affairs, Puducherry, my name has been mentioned as ''.
1. I am the deponent herein and I am residing at the abovesaid address.
2. I state that I was b o rn o n 2 8 -1 0 -19 6 9 to my parents namely, Soupramanien Caledonien and Andal Caledonien and during my birth, my name was spelled in French language as 'Saravanaperoumal Caledonien' and the same is entered in my Birth Certificate vide Registr atio n No. R/1969/00421, dated 29-10-1969.
3. Further, I state that I am working in the Filaria Control Unit (Health), Puducherry, as Field Worker and in the concerned Department Records, my name has been spelled in English language as 'Saravanaperumal'.
4 . Further, I state that the name spelled as 'Saravanaperoumal Caledonien' with French spelling and as 'Saravanaperumal' with English spelling are refer to one and the same person.
5. Further, I submit that this affidavit is being made in lieu proof of my name's identity as referred above.
