I state that my name is mentioned as 'Ashwin @ Vyshak' in my Secondary School Leaving Certificate issued by the State Board of School Examinations, Tamil Nadu, and in Transfer-cum-Conduct Certificate 'Ashwin Vyshak' due to numerological and religious reasons.
I hereby declare that henceforth I shall be known, called, referred and identified only as 'Ashwin Vyshak'.
I do hereby further undertake that I have not concealed any true facts or misrepresented and the facts stated by me above are true to the best of my knowledge, belief and information and nothing materials has been concealed thereon.
