2. I state that I was born on 13-4-1981 and named as '' by my parents and the same is entered in my Transfer Cer tificate in Admission No. 4232/1996-1997.
3. I state that my name is entered in my Aadhaar Card vide No. 4211 3837 2903, Elector's Photo Identity Card bearing No. GKY0397836 as 'Sivakumar ()'. But, my home-calling name '' is entered in my father's Family Ration Card bearing No. 202510 issued by the Department of Civil Supplies and Consumer Affairs, Puducherry.
4. Further, I declare that as per numerology, I have changed my name as 'Shivakumar ()' and I state that hereinafter, I shall be called and known only by the name as 'Shivakumar ()'.

I request that the correction may please be made and in future to avoid this error.
