My name is entered as 'Nehru S.T' in my Passbook issued by Karur Vysya Bank Limited, Karaikal.

In the Identity Card issued by the Public Works Department, Public Health Division under P. No. 479, my name has been mentioned as 'V. Souprayane @ Toulassidas'.

In the Joint Savings Account Passbook, vide No. 953324067 issued by Indian Bank, M.G. Road Branch, my name has been mentioned as 'Subrayane @ Toulassidas'.

In the Family Ration Card under No. 008946, issued by the Department of Civil Supplies and Consumer Affairs, Puducherry, my name has been mentioned as '•▪'.

I state that all the abovesaid names are refer, identify and relate to one and the same person that is myself the deponent herein.

Hereafter, my deceased father's name shall be 'Venougobalou dit Tirouvengada Poulle ( ▪)' and I shall be known and identified only by the name 'Souprayane dit Toulassidas (• ▪ )' for all purposes.

I declare that both the above names viz., 'Mohamed Tharick' and 'Mohamed Tharick. S' mentioned in the abovesaid records are denote one and the same person that is myself only.
