I, Kalpana Kousika, daughter of Radha Ramamoorthy, residing at No. 3, Main Road, Sivaranthagam Kizhur, Villianur, Puducherry-605 106, do hereby solemnly and sincerely affirm, to whomsoever it may concern, as follows:
I am the deponent herein. I state that in my Birth Certificate, under Registration No. M/1988/00421 and Elector 's Photo Identity Card under No. TLS0025148, my name has been mentioned as 'Kalpana @ Kousika'.
In my Aadhaar Card under No. 3561 9152 6512, in my Passport under No. S1874720 and in my State Bank of India Savings Account Passbook, my name has been mentioned as 'Kalpana Kousika'.
In my Family Ration Card under No. 089521, my name has been mentioned as ''.
I state that my daughter's name is 'V. Deepika' mentioned in my daughter's Aadhaar Card No. 8271 1145 7093.
I state that my daughter's name is 'Deepika, V' mentioned in my daughter 's School Record of Achariya Siksha Mandir.
I state that my daughter also known by her home-calling name as 'Priya (' and the same is in ad vertently mentio ned and evidenced in my Family Ration Card No. 089999.
I declare that all the above my daughter's names 'V. Deepika', 'Deepika. V.' and 'Priya (' are refer to one and the same person i.e., my daughter only.
Hereafter, my daughter shall be known only by the name 'Deepika' for all purposes.
