That I was born to my parents' namely, D. Ravi and Lactho umi Devy and they have named me a s 'Devi Ravi Narmadha'. And later on 16-09-2015, I had married one R. Siva Kumar S/o. Ramu.

That after the marriage, I have changed by my name as 'Narmadha Sivakumar'. I further submit that thereafter, and called and known as 'Narmadha Sivakumar' and hereinafter, I may be called and known as 'Narmadha Sivakumar'.

Further, I submit that my earlier name 'Devi Ravi Narmadha' and changed my name as 'Narmadha Sivakumar' (present name) is the name of one and the same person and that is myself.

I am executing this declaration to be submitted to the concerned authorities for the change of name.

I submit that the above stated lines are true and correct to the best of my knowledge and belief.
