I , Vijaya, wife o f Ramesh, Hindu, aged 36 years and residing at No. 9, 6th Cross Street, Muthamizh Nagar, Sedarapet, Puducherry-605 111, d o her eb y solemnly and sincerely affirm on oath as follows:

I am the deponent herein.

I submit that my name is mentioned as '➢↔' in my Family Ration Card No. 089565 issued by the Department of Civil Supplies and Consumer Affairs, Puducherry.

I submit that my name is mentioned as '↔' in my Marriage Invitation.

Further, I submit that my name is mentioned as 'Vijaya' in my School Transfer Certificate bearing Registration No. 011906 issued by the Headmaster, Government Higher Secondary School, Sedarapet, Puducherry and in my Aadhaar Card No. 2421 7617 3951 issued by the Unique Identification Authority of India and in my Voter's Identity Card No. MVM0211821 issued by the Election Commission of India.

I declare that the abovesaid names viz., '➢↔', '↔' and 'Vijaya' are denote one and the same person referring myself only.

Hereafter, I shall be known only by the name 'Vijaya'.

I hereby declare that I shall at all times hereafter in all reccords, deeds and writings and in all proceedings, dealings and transactions, private as well as upon all occasion whatsoever use and sign the name o f 'Vij aya' as my name in p lace and in substitution of my former name.
