I am the deponent herein. I state that in my Certificate of Birth issued by the Oulgaret Municipality, Puducherry, registered with it under Registration No. O/1962/00313, my name has been mentioned as 'Loganathan' and my father's name has been mentioned as 'Manikame'.

In my School Transfer Certificate under Admission No. 491 issued by the Headmaster, Government High School, Gandhi Nagar, Puducherry, my name has been mentioned as 'Loganathan M' and my father 's name has been mentioned as 'Manickam'.

In my Elector 's Photo Identity Card under No. PY/01/019/084419 issued by the Election Commission of India, my name has been mentioned as 'Loganathan' and my father 's name has been mentioned as Manickasamy'.

In my Aadhaar Card issued by the Unique Identification Authority of India under No. 8664 6122 3333, my name has been mentioned as 'M Loganathane' a n d m y f a t h e r 's n a m e h a s b e e n m e n t i o n e d as 'Manickasamy'.

In my Marriage Certificate registered under M.No.24/94, my name has been mentioned as M. Loganathan' and my father 's name has been mentioned as 'Manikkam'.

In the Judgement and Decree passed by the I Additional District Court at Puducherry in the Suit under O.S. No. 1671/2007, my name has been mentioned as 'Loganathane' and my father's name has been mentioned as 'Manicassamy @ Manikame'.

M. A D H A R S H, Commissioner. In my Family Ration Card under No. 415438 issued by the Department of Civil Supplies and Consumer Affairs, Puducherry, my name is mentioned as ''.

In my Birth Record of my deceased father under Registration No. O/1923/00315 registered with the Oulgaret Municipality, Puducherry, in the Marriage Certificate of my deceased father registered with t h e O u lg a r e t M u n ic i p al i t y, P u d uc h e r r y u nd e r No. OMI:25/1955 and in the Death Certificate of my father registered with the Oulgaret Municipality, Puducherry under Registration No. 2002/00299, my said father's name has been mentioned as 'Manicassamy'.

Therefore, I do hereby state that the abovesaid names viz., 'Manikame', 'Manickasamy', 'Manikkam', 'Manicassamy @ Manikame' and 'Manicassamy' are refer, identify and relate to one and the same person that is my deceased father and the said names viz., 'Loganathan', 'Loganathan. M', 'M. Loganathane', 'M. Loganathan', 'Loganathane' and '' are refer, identify and relate to one and the same person that is myself the deponent herein.
