I submit that my name is mentioned as 'Bakkialakshmi' in my Birth Certificate in Registration No. V/1982/00080, dated 20-2-1982 issued by Villianur Commune Panchayat, Puducherry, in my Aadhaar Card No.7080 6939 6658 and in my Elector's Photo Identity Card No. JQW0309708.
I submit that my name is mentioned as '' in my Family Ration Card No.444898.
Thus, both the names 'Bakkialakshmi' and '' are refer to one and the same person that is myself.
Hereafter, I shall be known only by the name 'Bakkialakshmi (•)'.
I hereby declare that I shall at all times hereafter in all records, deeds and writings and in all proceedings, dealings and transactions, private as well as upon all occasions whatsoever use and sign the name of 'Bakkialakshmi (•)' as my name in place and in substitution of my former name.
