I, Vijaylekshmi. V.S., daughter of V.P. Selvaraaj and S. Vaijayanthimala and wife of S.P. Ramkumar, residing at No. 123, Kandappa Street, Puducherry 605 001, Hindu, aged 37 years, do hereby solemnly and sincerely affirm and state as follows:
I am the deponent herein.
I state that I was born on 25th September, 1981 and registered my name as 'V.S. Vijaylekshmi' vide No. C/1981/1663 in records of the Registrar of Births and Deaths, Pondicherry Municipality, Puducherry and reflected in the Livret de famille-Family Record Book of my father as 'V.S. Vijaylekshmi'.
I state that in my Aadhaar Card issued by the Unique Identification Authority of India under Card No. 2236 8640 4578, my name has been registered as 'V.S. Vijaylekshmi'
I state that in my Family Ration Card No. 006267 issued to my father, by the Department of Civil Supplies and Consumer Affairs, Government of Puducherry, my name has been registered as '↔'.
I d o h e r e b y s t a t e t h a t t h e s a i d n a m e s 'V.S. Vijaylekshmi', 'V.S.Vijaylakshmi', 'Selvaraaj Vijaylekshmi', 'Vijaylekshmi' and '↔' are refer to one and the same person that is myself the deponent herein, identify and relate to one and the same person and I shall hereafter be known and identified only by the name 'Vijaylekshmi V.S.' for all purposes and intents.
