In my Aadhaar Card under No. 3687 7274 4288, in my Elector 's Photo Identity Card under No. HDF0213108 and in my PAN Card under No. AYEPP9910A, my name has been mentioned as 'Pemmadi Durga Prasad'.

D. ARUMUGAM, Commissioner. In my Family Ration Card under No. 16308 in Translation from Telugu Language to English, my name has been mentioned as 'Pemmadi Durga Prasad '.

Therefore, I do hereby declare that all the abovesaid names are refer, identify and relate to one and the same person that is myself, the deponent herein.
