3. Further, I submit that we are using his name as 'K.C. Dhev' for his identification in all places. The prefix 'K' to my son's name is the first letter of my name (i.e., Kiruchenamurthy) and the prefix 'C' to my son's name is the first letter of my wife's name (i.e., Chandra).

4. I humbly submit that due to religious reasons, we have prefixed the first letter of my wife's name 'C' (i.e., Chandra) also as one out of two initials, though in general, only the first letter of father's name 'K' (i.e., Kiruchenamurthy) will be prefixed as initial to the child.

I submit that as such the name 'Dhev' found in my so n's B ir th Certificate vide Registr atio n No. E/2015/01111 with Oulgaret Municipality and Aadhaar Card bearing No. 5388 5653 3528 and the name 'K. C. Mohan Raj', in my Aadhaar Card vide No. 7433 4071 3717 and in my Elector's Photo Identity Card, my name is mentioned as 'Mohanraj' vide No. DNX0380410, and in my Family Ration Card vide No. 11970, my name is mentioned as ''.

Whereas, my name has been mentioned as 'Mohan Raj Sakthivel' in my First Page (Bio-Data) of my Service Book of Government Central Press, Department of Stationery and Printing, Puducherry.

I declare and state that the names mentioned in the abovesaid records are denote and refer to one and the same person that is myself, the depondent herein. Henceforth, I shall be known, called and identified only by the name 'S. Mohanraj'.

Also, I state that my deceased father's name is mentioned as 'Sattiavelou' in my Birth Certificate vide Registration No. P/1978/00211 whereas, in my Aadhaar Card, Elector's Photo Identity Card, School Transfer Certificate and First Page (Bio-Data) of my Service Book of Government Central Press, my father's name has been mentioned as 'Sakthivel'.

Both the abovesaid names are refer to my deceased father only.

I state and declare that hereafter, I shall use my deceased father's name as 'Sakthivel'only.
