1. I declare that in my son's Birth Certificate, his name has been mentioned as 'Mugesh Kanna' issued by the Pondicherry Municipality, Puducherry, Registration vide No. M/2006/05788.

2. In my son's Aadhaar Card vide No. 2603 0395 1867, his name has been mentioned as 'Mugesh Kanna '.

3. In my son's Study Certificate issued by the Mo th er T he r esa H igher S eco nd a r y Sch o o l, Admission No. 883, his name has been mentioned as 'Mugeshkanna'

4. Whereas, in my Family Ration Card No. 436557, my son's name has been mentioned as '•  '.

Further, I declare that the above-mentioned names, viz., 'Mugesh Kanna', ' ', ' ', 'Mugeshkanna' and '•  ' are refer to one and the same person i.e., my minor son only.

Hereafter, I declare that my son may be called and known as 'Mugesh Kanna' only.
