In this connection I have consulted to Psychiatry who have examined me an opined that I will be benefited as a genuine male grace. Hence, I want to give full and free consent to be married as a man.

Followed by the surgery I am planning to change my indentify into a male including name change. I have also changed my name as 'Suhan'.

I am fully aware of the consequence of the gender reality of surgery the process as an incredible one.

In this context, I derive to state and marinate that, I am not indigested or influenced by any person regarding the conversance and I have given free consent for undergone operation apart if any, risk to myself in undergoing the operation. I stand and hold liable to bear that I am fully responsible by submitting this affidavit solemnly and sincerely affirmation on oath.
