Further, I state and declare that in my PAN Card, my name is spelled as 'Kannaki' and the same name is mentioned as '' in my Family Ration Card.
Further, I state and declare that since, I got one nick name i.e., 'Aruna' the same is entered in my Elector's Photo Identity Card.
Therefore, I declare that the aforesaid names i.e., 'Canaguy', 'R. Kannaki', 'A Kannagi', 'R. Kannahi' 'Kannaki', '' and 'Aruna' are referred one and the same person i.e., myself.
Further, I declare that hereafter, I shall be called in my OCI Card No. A3476146, my name is spelt co rrectly as 'Zakier Hussain'. However, in my Educational Records, my family initial is prefixed to my name and mentioned as 'A.M.S.J. Zakier Hussain'. The initials A.M.S.J. prefixed to my name 'Zakier Hussain' denote the name of my ancestors on paternal side. I hereby declare that the names 'Zakir Hussain', 'Zakier Hussain' and 'A.M.S.J. Zakier Hussain' as found differently spoken in the above referred documents are to me alone and none else. In order to ensure uniformity of my identity, I shall henceforth be known, called, identified and referred only as 'Zakier Hussain' and not by any other name.
