8. That my name has been registered in my Civil Marriage Registration Certificate under Registration No. 122/2019/PMC issued by the Sub-Registrar, Pondicherry Municipality, Puducherry as 'Shakthisree'.

9. That my name has been recorded in my Religious Convertion in Certificate C.No. 10/2020 issued by the Naib Kazy, Puducherry as 'Aisha'.

10. That my name has been registered in my Muslim Traditional Marriage Registration Certificate under Marriage Register Volume No. 1, Sheet No. 35, issued by the Naib Kazy, Puducherry as 'Shakthisree @ Aisha'.

Hence, all the abovesaid names such as 'Shakthisree', 'M Shakthi Sree', 'Shakthisree M', 'Aisha', 'Shakthisree @ Aisha', '  ' and '' are referring one and the same person i.e., myself only. I furnish this affidavit towards my names' identity/proof and that all the names are belonging and referred one and the same person i.e., myself only.
