The objections so received within the specified period will be considered by the Villianur Commune Panchayat.

My name is mentioned in my abovesaid Passport as 'Mohammad Asaad Raza' with my surname as 'Mohammed Moinuddin'. Now, I wish to relinquish my surname. Hereafter, I will be known as 'Mohammad Asaad Raza' without my surname.

This affidavit is made for Gazette publication to omit my surname in my said Indian Passport.

I am making this affirmation in order to delete my surname 'Mohammad Moinuddin' from my Indian Passport and to enter my correct name as 'Mohammad Asaad Raza'.
