In my daughter Amelia Mahimainathan's Secondary School Examination Certificate under Sr. No. of Statement 187105 and Higher Secondary Certificate Examination under Sr. No. of Statement 172538, my name has been mentioned along with her name as 'Amelia Mahihainathan'.

In the Aadhaar Card of my daughter under No. 5795 1875 4024 and in the Transfer and Conduct Certificate under TC No. 2K14/14MCA402, my name has been mentioned along with her name as 'Amelia Mahimainathan'.

In the PAN Card of my abovesaid daughter under No. BWTPA9988F, my name has been mentioned as 'Mahimainathan'.

Therefore, I do hereby declare that the said names 'ƒ \˛Á\ÂV>[', 'S Mahimainathan', '\˛Á\ÂV>[', ' M a h i m a i n a t h a n ' , ' M a h i m a i n a t h a n S ' a n d 'Mahihainathan' are reffered, identified and relate to one and the same person that is me, the deponent herein.

I state that hereafter, I shall be known and identified only by the name 'Mahimainathan (\˛Á\ÂV>[)' for all purposes.

ADDENDUM

In the G.O. Ms. No. 11/Fy., dated 07-08-2020 from the Under Secretary to Government (Fisheries), Chief Secretariat, Puducherry, a Union Territory Level Approval and Monitoring Committee (UTLAMC) was constituted in the Union territory of Puducherry for smooth implementation of Pradhan Mantri Matsya Sampada Yojana (PMMSY).

2. Whereas, vide Letter No. J-117012-2/2020-Fy., dated 28-09-2020 of the Under Secretary to Government of India, Department of Fisheries, Ministry of Fisheries, Animal Husbandry and Dairying, New Delhi, the Government of India has decided to include the Chief Executive Officer (CEO)/Mission Director, National Rural Livelihood Mission (NRLM) in the State/Union Territory as one of the Member of the SLAMC/UTLAMC for better coordination and convergence.

3. Now, therefore, in compliance, the Lieutanent- Governor, Puducherry, is pleased to include the Chief Executive Officer, Puducherry State Rural Livelihood Mission, Puducherry-cum-Assistant State Project Director, DRDA, Puducherry, as one of the Member of the Union Territory Level Approval and Monitoring Committee (UTLAMC).

4. All other terms and conditions in the G.O. Ms. No. 11/Fy., dated 07-08-2020 from the Under Secretary to Government (Fisheries), Chief Secretariat, Puducherry, remain unaltered.

5. This issues with the concurrence of the Finance Department, Puducherry vide I.D. No.1474/FC/FD/F4/ A3/2020-21, dated 10-02-2021.
