I, Seyed Ahamed, son of Segou Mougamadou and Djeinambou, aged about 89 years and residing at No. 8, Singara Velanar Street, Karaikal-609 602, Union territory of Puducherry, do hereby solemnly and sincerely affirm on oath and declare as follows:
That the name of my deceased father has been mentioned as 'Check Mohamed' in my mother Djeinambou's Death Registration No. 232/1963 and his name is mentioned as 'Check Mohamed marecar' son of 'Nour Mohamed' in his Death Registration No. 412/1962, and his name is mentioned as 'Segou Mougamadou' son of 'Nour mougamadou' in his Birth Registration No. 231/1878 that all the documents are registered at Karaikal Municipality, Karaikal.
I hereby state that all the names of my deceased father namely, 'Check Mohamed', 'Check Mohamed marecar' son of 'Nour Mohamed' and 'Segou Mougamadou' son of 'Nour Mougamadou' entered on the abovesaid records are denoting one and the same person that is my deceased father 'Segou Mougamadou' only.
The above statement is true and correct to the best of my knowledge and belief, and nothing material has been concealed therein.
