I, Manjamatha, daughter of Munusamy and wife of Sateshkumar, aged about 33 years and residing at No. 110, Mariamman Koil Street, Kundupalayam, Puducherry-605 009, do hereby solemnly and sincerely affirm on oath as follows:

That I am the deponent herein. I submit that my name is mentioned as 'Manjamatha' in my Birth Certificate in Registration No. M/1988/07832, dated 31-10-1988 issued by the Pondicherry Municipality, Puducherry and in my Elector's Photo Identity Card No. KVY0687509 issued by the Election Commission of India.

I submit that my name is mentioned as 'Manjamatha. M' in my School Transfer Certificate No. 44075 issued by the Government Girls' Higher Secondary School, Kathirkamam, Puducherry.

I submit that my name is mentioned as 'Manjamatha M' in my Secondary School Leaving Certificate No. SEC 2462985 issued by the State Board of School Examinations, Tamil Nadu.

I s u b m i t t h a t m y n a m e i s m e n t i o n e d a s 'S Manjamatha' in my Aadhaar Card No. 6466 8361 5043 and in my PAN Card No. FNAPM1202C.

I s u b m i t t h a t m y n a m e i s m e n t i o n e d a s 'Manja Matha' in my daughter Mogana Pragathi's Birth Certificate in Registration No. O/2013/25, dated 18-01-2013 issued by the Oulgaret Municipality, Puducherry.

I s u b m i t t h a t m y n a m e i s m e n t i o n e d a s 'M. \ﬁƒ\V>V (®) \ﬁ∑·V' in my Marriage Invitation Card.

I s u b m i t t h a t m y n a m e i s m e n t i o n e d a s '\ﬁ∑·V' in the Family Ration Card No. 343093 issued by the Department of Civil Supplies and Consumer Affairs, Government of Puducherry.

Thus, all the names viz., 'Manjamatha', 'Manjamatha. M', 'Manjamatha M', 'S Manjamatha', 'Manja Matha', 'M. \ﬁƒ\V>V (®) \ﬁ∑·V' and '\ﬁ∑·V' are referring one and the same person that is myself only.

Hereinafter, I shall be known and identified only by the name 'Manjamatha'.

I hereby declare that I shall at all times, hereinafter in all records, deeds and writings, and in all proceedings, dealings and transactions, private as well as upon all occasions, whatsoever use and sign the name of 'Manjamatha' as my name in place and in substitution of my former name.

That I am the deponent herein.

I hereby state that according to my Birth Certificate vide Registration No. P/1965/00279, dated 21-01-1965 issued by Pondicherry Municipality, Puducherry, my name is mentioned as 'Vassantha'.

Further, I submit that in my Aadhaar Card No. 8656 3274 0831, my name is mentioned as 'Vassantha'.

I also state that in my Electoral Identity Card vide Serial No. NOG0201145, my name is mentioned as 'Vassantha'.

Whereas, in the Family Ration Card vide Serial No. 073945 issued by the Department of Civil Supplies and Consumer Affairs, Puducherry, my name is mentioned as 'g]Èit' instead of 'Vassantha'.

Further, I state that in my son Manoharan's Marriage Invitation, dated 08-09-2021, my name is mentioned as 'g]È'∑t (®) kƒÕ>V ✓È´V\['.

Therefore, I do hereby declare that all the above-mentioned names are referred, identified and relate to one and the same person that is myself only.

I state that hereinafter, I shall be known and identified only by the name 'Vassantha (kƒÕ>V)' for all purposes.

The above facts are true to the best of my knowledge and belief.

I state that my name has been written as 'Sivasankar' in my Higher Secondary Course Mark Certificate under No. HSG 5143262, Register No. 546129 and TRM Code No. G482288, dated 14/05/2010, 10th Standard School Transfer Certificate issued by the Vivekanandha Higher Secondary School under Admission No. 2187, Serial No. 5757/09-10, dated 19-05-2010, Transfer- cum-Conduct Certificate issued by the Sri Manakula Vinayagar Engineering College, Madagadipet, Puducherry under Serial No. SMVEC/TC/EEE/ 2013-14/105, Admission No. EE1086, dated 09-07-2014, Aadhaar Card No. 4711 6527 4640 and in my Elector's Photo Identity Card No. ATT0061911.

I state that for numerological and sentimental reasons, I have now changed my name as 'James Sivasankar Soupane' and hereinafter, I shall be known by the new name as 'James Sivasankar Soupane' on all records, papers and all affairs of my life.

The above deposition is true and correct to the best of my knowledge, belief and nothing material has been concealed therein.

I was a Hindu by birth. My birth name was 'Sundaramoorthy'. Recently, I had embraced Islamic faith. Upon my conversion of religion, I had renounced my birth name 'Sundaramoorthy' and assumed to myself the Muslim name 'Saleem'.

I hereby declare that henceforth, I shall be known, called, referred and identified only as 'Saleem'.
