I state that my name has been mentioned as 'Pramila' in my Aadhaar Card No. 5124 9525 0687 and my name has been mentioned as 'Pramila (✓´t·V)' in Family Ration Card and in my Elector's Photo Identity Card.

I state that my name has been mentioned as 'Premila' instead of 'Pramila' in my Husband's Office Record who is working as a Lab Attendant, Office of the Additional Director of Agriculture (LUP & QC), Thattanchavady, Puducherry, who died while in service on 07-06-2022.

Further, I declare that both the names 'Pramila' and 'Premila' are referring one and the same person i.e., myself only.

ERRATUM

The notification vide G.O. Ms. No. 64, dated 29th June 2022 of the Chief Secretariat (Works), Puducherry, p u b l i s h e d i n t h e O f f i c i a l G a z e t t e N o . 28, d a t e d 12th June 2022, in the said notificaton, the name "P. Balasubramanian" has been published instead of "B. Balasubramanian". Now, it is corrected and read as "B.
