Further, I state and declare that in view of the judgment for the divorce in between me and father of the said child, I intend to put the initial of my son/child as 'I. Yashwin Vishwak'.

Therefore, I declare that hereinafter the initial of my son is 'I. Yashwin Vishwak' only, thus, I have executed this affidavit, in this regard, I have not concealed any real facts.
