I state that my name is 'Jagatheesh' which is entered in my Birth Certificate born on 28-02-1984 and registered before the Registrar of Births and Deaths, Villianur Commune Panchayat, Puducherry vide Registration No.VC/T/1984/000053, registered on 03-03-1984, in my daughter's Birth Certificate namely, Rithika, born on 07-06-2006 and registered before the Registrar of Births and Deaths, Oulgaret Municipality, Puducherry vide Registration No. OM/K2006/000751, dated 26-06-2006 and in my son's Birth Certificate namely, Aatish born on 22-07-2010, registered before the Registrar of Births and Deaths, Oulgaret Municipality, Puducherry vide Registration No. OM/ J/2010/008516, dated 29-07-2010 and in my PAN Card under No. APBPJ8729D issued by the Income-tax Department, Government of India, in my Aadhaar Card Under No. 6218 5970 2543 issued by the Unique Identification Authority of India, my name has been entered as 'R.Jagatheesh' in my India Driving Licence issued by the Assistant Licensing Authority, Tindivanam under No. TN32Y20090002412, dated 19-08-2009, my name has been entered as 'Jagatheesh R' and in my Family Ration Card under No. 189944 issued by the Department of Civil Supplies and Consumer Affairs, Puducherry my name has been entered as 'ÿ¤Ô]¸'.

Finally, I declare that hereinafter, I shall be called and known only by the name 'Jagatheesh' in all correspondences, records, etc., in future.

I state that what are all stated in the above paragraphs is true and correct to the best of my knowledge, belief and information.

Further, I submit that in my daughter Rithika's Marks Statement-cum-Certificate issued by the Central Board of Secondary Education vide No. 2073343, dated 22-07-2022 and the Transfer Certificate issued by the Principal, Kendriya Vidyalaya-2, Kalapet, Puducherry under No. 63645, dated 28-07-2022, my name has been entered as 'Jagadish'.
