That I am the deponent herein and I well known all the facts of my deposition.
I submit to state that my name is mentioned as 'Balachandar @ John Fernandes' in my son's Birth Certificate issued by the Pondicherry Municipality, Puducherry vide Registration No. M/2008/06941, dated 02-06-2008 and in his bona fide Certificate issued by the Principal, Petit Seminaire Higher Secondary School, Puducherry-605 001, dated 5-9-2022 and in my Marriage Certificate issued by the Pondicherry Municipality, Puducherry, dated 26-07-2021.
I submit to state that my name is mentioned as 'Balachandar' in my Aadhaar Card vide No. XXXX XXXX 2619 issued by the Unique Identification Authority of India
I submit to state that my name is mentioned as 'Balachandar. V' in my Transfer Certificate issued by the Principal, Pope John Paul II College of Education vide No. 1999-2000/14103.
I submit to state that my name is mentioned as '✓VÈflƒÕ>Ï' in the Family Ration Card issued by Department of Civil Supplies and Consumer Affairs, Puducherry vide No. 060771.
Finally, I declare that the abovesaid names 'Balachandar @ John Fernandes', 'Balachandar. V', 'Balachandar' and '✓VÈflƒÕ>Ï' are referring one and the same person, they denoting myself only.
Henceforth, I will be called, known and signed as 'Balachandar V' only.

R' i n my School Certificate, 'Raju Ponnuvelu A Anthony' is mentioned in my Family Ration Card, 'Raju Ponnuvelu' is mentioned in my Discharge Certificate, Ex-servicemen Identity Card, Aadhaar Card, PAN Card, Elector's Photo Identity Card, my Bank Account in IOB, Driving Licence and also in my wife's Passport (named of spouse column).
I declare that all the abovesaid names 'Raju Ponnu Velu', 'Raju Ponnuvelu A Anthony', 'Ponnuvelu. R' and 'Raju Ponnuvelu' are referring one and the same person that is myself only.
