[697]712 LA GAZETTE DE L'ETAT [26 September 2023 In conformity with rule 11 of the Puducherry Commune Panchayats (Grant of Licences and Permissions) Rules, 1976, promulgated of this Union Territory by the Puducherry Village and Commune Panchayats Act, 1973, objections, if any, to the setting up of the above proposed industry, are invited to reach the Commissioner, within ten days from the date of publication of this notice in the Official Gazette.

The objections received within the specified period will be considered by the Nettapakkam Commune Panchayat.

AFFIDAVIT

I, Sunitha, wife of R. Vadivelou, and residing at No. 34, Thottakkal Main Road, Karamanikuppam, Mudaliarpet, Puducherry-605 004, do hereby solemnly and sincerely affirm, and state on oath as follows:

That I am the deponent herein and well aware of the facts of the deposition.

I state that my daughter namely, 'Sasmitta Ronsard' was born on 08-12-2008 at Maternity Hospital, Puducherry, which is duly registered at Pondicherry Municipality, Puducherry vide Registration No. PM/ M/2008/016892, dated 10-12-2008.

I state that my name is entered as 'Ronsard Sunitha' in her abovesaid Birth Certificate.

I state that I am called as 'Sunitha' by my relatives and others and do not possess any evidence for the documentary proof in support of my name.

Futher, I state that my name is entered as '∑M>V' in my Family Ration Card No. 478854, issued by the Department of Civil Supplies and Consumer Affairs, Puducherry.

Therefore, I do hereby declare that my names i.e., 'Sunitha' and '∑M>V' are referred, identified and relate to one and the same person that is myself only.

Hereinafter, I shall be known and identified only by the name 'Sunitha' for all purpose.

I state that in my daughter's abovesaid Birth Certificate, my name is mentioned with my mother's family name and mentioned as 'Ronsard Sunitha'.

I state that my name is entered as 'Sunitha' in my Birth Certificate vide Registration No. M/1983/05053, dated 09-08-1983, issued by Pondicherry Municipality, Puducherry and in my son Subhikshan's Birth Certificate vide Registration No. E/2013/02752, dated 23-04-2013, issued by Oulgaret Municipality, Goverment of Puducherry.

I state that my name is mentioned as V.Sunitha in my Aadhaar Card No. XXXX XXXX 5495, issued by the Unique Identification Authority of India, further, I state that my name is entered as 'S. Sunitha' in my SSLC Mark Sheet, after my marriage, I have used my name as 'V. Sunitha' in all records of Aadhaar Card, Bank Passbook and Family Ration Card No. 478854, issued by the Department of Civil Supplies and Consumer Affairs, Puducherry.

Therefore, I do hereby declare that these names, i.e., 'Ronsard Sunitha', 'Sunitha', 'V.Sunitha' and 'k.∑M>V' are referred, identifed and relate to one and same person that is myself only. Hereafter, I shall be known and identified only by the name 'Sunitha' for all purpose.

I hereby declare that I shall at all times hereinafter in all records, deeds and writings and in all proceedings, dealings and transactions private as well as upon all occasions whatsoever use and sign my name 'Sunitha' in place and in substitution of the former names. 6. In my PAN Card bearing No. FSBPP3784E, my name is mentioned as 'S.Palaniyandi'.

Further, I state that due to numerological reasons, I proposed to change my name as 'Palani'.

Therefore, I declare that hereinafter, I shall be known and called only as 'Palani'.

Further, I state that my deceased father's name 'Aroumougame' as found in my Aadhaar Card, Birth Certificate and 'Arumugam' as found in my Transfer Certificate are referring one and the same person i.e., my deceased father only.
