4. In my Bank Passbook of Indian Bank, Seliamedu Branch, Puducherry, in Savings Account No. XXXXXX2218, in which my name is spelled as 'Sathiyaseelan'.

5. In my Aadhaar Card, issued by Unique Identification Authority of India under No. XXXX XXXX 1119, in which my name is spelled as 'Sathiyaseelan (ƒ›]BÊÈ[)'.

6. In my Elector's Photo Identity Card, issued by Election Commission of India under No. GHN1001924, in which my name is spelled as 'Sathiyaseelan (ƒ›]BÊÈ[)'.

8. In my Sale Deed, dated 11-08-2010 registered at Thirupur District under Document No. 14221/2010 in which my name is spelled as 'P.ƒ›]BÊÈ['.

9. In my PAN Card, issued by Income-tax Department under No. OMVPS4211Q, in which my name is spelled as 'Sathiyaseelan'.

10. In my Driving Licence, issued by Tamil Nadu Transport Department No. TN66 20130003609, in which my name is spelled as 'Sathiyaseelan P'.

11. In my Marriage (Nikkah) Certificate, issued by Puducherry Orleanpet Masjith Mohamadia Al Sunnathul Valjamath Mosque, dated 31-10-2011, in which my name is spelled as '∂©m_ ´„\V[ (®) ƒ›]BÊÈ['.

12. In my son Raashith Ahamed's Birth Certificate born on 19-10-2012, issued by Oulgaret Municipality, Puducherry under Registration E/2012/09/99, in which my name is spelled as 'Sathiyaseelan @ Abdul Rahaman'.

13. In my son Raashith Ahamed studying in Immaculate Heart of Mary's Government Aided Boys' and Girls' High School, Nellithope, Puducherry, in which my name is spelled as 'Sathiyaseelan @ Abdul Rahaman'.

14. I, voluntarily and out of my own willing, embrace Islamic religion before Government Town Kazy at Puducherry on 31-03-2011, my name is mentioned as 'Abdul Rahman (∂©m_ ´„\V[)'. 15. Often, I met difficulties to explain the differences in my name's spelling in the above-mentioned documents and I hereby declare that all the names viz., 'Sathiyaseelan, 'ƒ›]BÊÈ[', 'P.ƒ›]BÊÈ[', Sathiyaseelan.P', '∂©m_ ´„\V[ @ ƒ›]BÊÈ[', 'Abdul Rahman (∂©m_ ´„\V[) and 'Sathiyaseelan @ Abdul Rahaman' are denoting one and the same person, they referring myself only.

16. Further, I declare that hereinafter, I shall be known and identify only by the name 'Abdul Rahaman (∂©m_ ´„\V[)'.

17. Nothing is concealed in this affidavit and Further, I sworn in that any of the contents of this affidavit found false, I shall undertake to face any legal consequences. The contents of this affidavit are true and correct to the best of my knowledge.

But, in my School Transfer Certificate Admission No. l845 and S.S.L.C. Mark Sheet Registration No. 477520, my name has been mentioned as 'P Gopinath'.

Hence, both the names 'P. Arulraj @ Gopinath' and 'P Gopinath' are denoting one and the same person i.e., myself only.

Hence, I declare my correct and proper name is 'P. Arulraj Gopinath' only.

Hence, the above affirmation is made in lieu proof of my correct name and the same is produced before the concern authority for my names' identity.

Whatever stated in the above affidavit are true and correct to best of my knowledge and belief, and nothing material is concealed herein.

Therefore, I swear this affidavit for the knowledge of my relatives, friends, officials concerned and others that hereinafter, my son will be known only as 'Muhammed Shamil Selahuddin' in all purpose and intends after the publication of the Official Gazette.
