I state that my correct name is mentioned as 'Cartigue Coumary ('க்ஷிரூழ்]று' க்ஷ்\க்ஷித்)' in my Birth Certificate bearing Registration No. PM/P/1958/1562, issued by Pondicherry Municipality, Puducherry and in my Aadhaar Card No. XXXX XXXX 1526, issued by Unique Identification Authority of India.

I state that due to inadvertence I have given my name mistakenly and entered as 'ணிழ்டுக்ஷி' in my Family Ration Card No. 027380, issued by the Department of Civil Supplies and Consumer Affairs, Puducherry and the said name has been called by my family members, relatives and others.

I do hereby declare that the abovesaid names i.e., 'Cartigue Coumary ('க்ஷிரூழ்]று' க்ஷ்\க்ஷித்)' and 'ணிழ்டுக்ஷி'are referring and denoting one and the same person, i.e., myself only. Further, I state that my correct name and henceforth I shall be known only as 'Cartigue Coumary ('க்ஷிரூழ்]று' க்ஷ்\க்ஷித்)'.

I declare that what are all stated above is true and there is no suppression or misrepresentation of the facts.
