That I was a Hindu by birth and my birth name is 'Sakthimohan'. Due to my change of religion from Hinduism to Islamic faith I had renounced my birth name 'Sakthimohan' and assumed to myself the Muslim name 'Muhammathu Thowfeek (ஜ்ஞிே\னீ ஸ்ரீ>·ண்ூ)ை'. I hereby relinquish my birth name 'Sakthimohan' and hereafter, I shall be known, called, identified and referred to only as 'Muhammathu Thowfeek'.
