I, Syed Ahamed Kabir Rafeek, son of Mohamed Arib Maricar, Indian, residing at No. 71, Kailasanathar Koil Street, Karaikal town and District Munsifi, do hereby solemnly affirm and state on oath as follows:

1. In my Birth Certificate issued by Karaikal Municipality vide Registration No. K/1982/01975, my name has been entered as 'Syed Ahamed Kabir Rafeek Maricar'.

2. In my Aadhaar Card, issued by Unique Ide nti fi cat ion Aut ho ri ty of I nd ia vi d e Car d No. XXXX XXXX 1252, my name has been entered as 'Syed Ahamed Kabir Rafeek'.

3. In my PAN Card, issued by Income-tax D e p a r t m e n t , G o v e r n m e n t o f I n d i a , C a r d No. BXFPS3501K, my name has been entered as 'Syedahamedkabirrafeek'.

4. In my Elector's Photo Identity Card, issued by Election Commission of India vide No. KSQ0234773, my name has been entered as 'Syed Ahamed Kabir Rafeek ('.

5. In my Marriage Certificate, issued by Karaikal Municipality vide Registration No. 311/D.M.2010, my name has been entered as 'Syed Ahamed Kabir Rafeek Maricar'.

6. Whereas, in my Family Ration Card, issued by the Department of Civil Supplies and Consumer Affairs, Karaikal vide Card No. 197656, my name has been entered as ''.

7. Therefore, I do hereby declare that all the abovesaid names i.e., 'Syed Ahamed Kabir Rafeek Maricar ', 'Syed Ahamed Kabir Rafeek', 'Syedahamedkabirrafeek', 'Syed Ahamed Kabir Rafeek (   ', 'Syed Ahamed Kabir Rafeek Maricar' and '' are referring one and the same person i.e., myself only.

8. Hereinafter, I shall be called, identified by the name as 'Syed Ahamed Kabir Rafeek' only.

That I am the deponent herein.

I state that my name is 'Arthanari' and the same is found in my Marriage Certificate, Indian Passport, PAN Card, and in the Birth Certificate of my sons, 'Sitheshwaran and Kodiswaran', respectively.

I state that my name is mentioned as 'Arthanari Sivachariyar ' is found in my Aadhaar Card and Elector's Photo Identity Card respectively.

I do hereby declare that the names 'Arthanari' and 'Arthanari Sivachariyar' are referring one and the same person i.e., myself only.

In the G.O. Ms. No. 38/2023-LD., dated 15-11-2023 of the Law Department, Government of Puducherry, published in the Official Gazette No. 48, dated 28-11-2023, fo r the existing para 1 and the Tab le, the following shall be substituted, namely:-

"Sanction of the Lieutenant-Governor is hereby conveyed for the creation of 2 posts of System Analyst (Group-B Gazetted) and 6 posts of Assistant Programmer (Group-C) in the Judicial Department, Union territory of Puducherry, as detailed below:
