I state that my name is entered as 'Sivagami Alias Sivasangari' in my husband's Death Certificate, issued by Thirunallar Commune Panchayat, Thirunallar, Karaikal, bearing Registration No. TC/T/2025/00026, dated 01-02-2025.

I state that my name is entered as 'Sivasankari V' in my Aadhaar Card, issued by Unique Identification Authority of India, bearing No. XXXX XXXX 0472.

I state that my name is entered as 'Sivasankari' in my Elector's Photo Identity Card, issued by Election Commission of India, bearing No. URI0181347.

C. RAMESH, Commissioner. I state that my name is entered as 'P. Sivakami' in my Transfer Certificate, issued by the Headmistress, Government Girls Higher Secondary School, Mayuram, bearing Admission No. 8904.

I state that in my husband's Pension Payment Order, my name is entered as 'V Sivagami', issued by Directorate of Accounts and Treasuries, Puducherry, bearing Pension Payment Order PPO No. 14168.

I state that my name is entered as '▪ ' in my Family Ration Card, issued by the Department of Civil Supplies and Consumer Affairs, Karaikal, bearing No. 206925.

Hence, I submit that the names described in the abovesaid records viz., 'Sivagami Alias Sivasangari', 'Sivasankari V', 'Sivasankari', 'P Sivakami', 'V Sivagami' and '▪' are referring one and the same person, i.e., myself only.
