In conformity with rule 11 of the Puducherry Commune Panchayats (Grant of Licences and Permissions) Rules, 1976, promulgated in this Union Territory by the Puducherry Village and Commune Panchayats Act, 1973, objections, if any, to the setting up of the abovesaid industry, are invited to reach the undersigned within ten days from the date of publication of this notice in the Official Gazette.

The objections, so received within the specified period will be considered by this Commune Panchayat.

I, Usharani, wife of George Ravi, Christian, aged about 64 years, residing at No. 15, Kottucherry Road, Keezha Nedungadu, Nedungadu Commune, Karaikal-609 603, do hereby solemnly affirm and state as follows:

That I am the deponent herein and made this affidavit on behalf of myself.

I state that my name is entered as 'Oucharany' in my Birth Certificate, issued by Nedungadu Commune Panchayat, Karaikal, Registration of Births and Deaths bearing Registration No. NG/N/1961/000082, dated 09-06-1961.

I state that my name is entered as 'Usharani Geroge Ravi' in my Aadhaar Card, issued by Unique Identification Authority of India bearing No. XXXX XXXX 0974.

I state that my name is entered as 'Oucharany' in my Elector 's Photo Identity Card, issued by Election Commission of India bearing No. SJU0199562.

I state that my name is entered as 'Mary Regina Alias Usha Rani' in my Marriage Certificate, issued by Nedungadu Commune Panchayat, Karaikal bearing Registration No. 32, dated 12-11-1999.

I state that my name is entered as '➢▪ ' in my Family Ration Card, issued by the Department of Civil Supplies and Consumer Affairs, Karaikal bearing No. 416443.

I state that my name is entered as 'P Usharani' in my Bank Passbook, issued by Indian Overseas Bank, Nedungad u B ranch, Ka raika l bear ing Account No. XXXXXXXXXXX5302.

Hence, 1 submit that all the names described in the abovesaid records viz., 'Oucharany', 'Usharani Geroge Ravi', 'Mary Regina Alias Usha Rani', 'P Usharani' and '➢ ▪ ' are referring one and the same person, i.e., myself only.

Further, I state that hereinafter, for all my official and private purposes, I shall be called, known and distinguished only by the name as 'Usharani'.

I state that I have renounced Hindu religion and embraced Buddha religion, as such in the Bauddha Dhamma Diksha Certificate, issued by the Buddhist Society of India, dated 14-04-2019, my name has been mentioned as 'P. Pakkialatchoumy'.
