I state that my name is entered as 'D Banumathi' in my Savings Bank Account of UCO Bank vide Account No. XXXXXXXXXX7060.

I state that my name is entered as '' in our Family Ration Card, issued by the Department of Civil Supplies and Consumer Affairs, Puducherry bearing No. 063856.

Hence, I submit that the names described in the above records viz., 'Banoumady', 'Banumathi', 'Banoumady R', 'Banumathy' and 'D Banumathi' are referring and denoting one and the same person, i.e., me and my name only.

Finally, I declare that my correct name is 'D. Banoumady' only.
