The undermentioned person has requested for permission to set up the following industry within the Mannadipet Commune Panchayat limits as detailed below.

In conformity with rule 11 of the Puducherry Commune Panchayats (Grant of Licences and Permissions) Rules, 1976, promulgated in this Union Territory by the Puducherry Village and Commune Panchayats Act, 1973, objections, if any, to the setting up of the above industry, are invited to reach the Commissioner, Mannadipet Commune Panchayat within ten days from the date of publication of this notice in the Official Gazette.

The objections, so received within the specified period will be considered by this Commune Panchayat.

I, Sai Suruthi, wife of V. Sundheep Kumar, daughter of P. Muralidhar, aged 28 years, residing at No. 7, Block 8, Jeevanandam Street, Ashok Nagar, Lawspet, Puducherry- 605 008, do hereby solemnly and sincerely affirm, and state on oath as follows:

That I am the deponent herein. I am well aware of the facts of my deposition.

1. I state that my name is mentioned as 'Saishruthi' in my Birth Certificate bearing Registration No. C/1997/ 03559, issued by Pondicherry Municipality, Puducherry.

2. I state that my name is mentioned as 'M Sai Shruthi' in my Aadhaar Ca rd vide No. XXXX XXXX 1251, issued by Unique Identification Authority of India.

3. I state that my name is mentioned as 'M Sai Shruthi' in my PAN Card No. MMWPS9594M, issued by the Income-tax Department, Government of India.

4. I state that my name is mentioned as 'Sai Shruthi' in my Elector 's Photo Identity Card vide No. IPD0070920, issued by Election Commission of India.

5. I state that my name is mentioned as 'Sai Suruthi' in my Employment Identity Card vide Registration No. PY011201312287, issued by Employment Exchange, Puducherry.

8. I state that my name is mentioned as 'Sai Suruthi.M' in my B.D.S Degree College Transfer-cum-Conduct Certificate vide Admission No. 1039, issued by Mahatma Gandhi Postgraduate Institute of Dental Sciences, Puducherry.

9. I state that my name is mentioned as '' in our Family Ration Card vide No. 025355, issued by the Department of Civil Supplies and Consumer Affairs, Puducherry.

Hence, I declare that these names i.e., 'Saishruthi', 'M Sai Shruthi', 'Sai Shruthi', 'Sai Suruthi', 'Sai Shruthi M', 'Sai Suruthi M', 'Sai Suruthi.M' and '' are referring one and the same person, i.e., myself only.
