I state that my name is entered as 'Punidavally' in my Birth Certificate, issued by Neravy Commune Panchayat, Karaikal bearing Registration No. NE/N/ 1980/145, dated 01-10-1980.

I state that my name is entered as 'Punitha Sudhakar' in my Aadhaar Card, issued by Unique Identification Authority of India bearing No. XXXX XXXX 1857.

I state that my name is entered as 'Punidavally' in my Electoral Identity Card, issued by Election Commission of India bearing No. NPX0228882.

I state that my name is entered as 'Punidhavalli G' in my Transfer Certificate, issued by the Headmaster, M.A. Government Girls' High School, Karaikal bearing Sl. No. 59258/58/98-99.

In my daughter '•' Birth Certificate vide Registration No. 5271, my name is mentioned as ''.

In my Family Ration Card No. 431995, my name is mentioned as ''.

In my Caste Certificate C.No. 976/B5/TLK/KKL/ 2001, dated 24-01-2002, my name is mentioned as 'Punithavally'.

The abovesaid names are referred one and the same person they and denote me and myself only.

Finally, I declare that my correct name is 'Punidavally' only. Further, I will not insist any correction in future.

The above statements in the foregoing paragraphs are true to the best of my knowledge and belief.
