In conformity with rule 11 of the Puducherry Commune Panchayats (Grant of Licences and Permissions) Rules, 1976, promulgated in this Union Territory by the Puducherry Village and Commune Panchayats Act, 1973, objections, if any, to the setting up of the above industry, are invited to reach the Commissioner, Nettapakkam Commune Panchayat within ten days from the date of publication of this notice in the Official Gazette.

The objections, so received within the specified period will be considered by this Commune Panchayat.

I, Parthiban, son of Rajendiran, Hindu, aged about 31 years, residing at No. 65, Main Road, Varichikudy (North), Kottucherry Commune, Karaikal District, do hereby solemnly affirm and state as follows:

That I am the deponent herein and made this affidavit on behalf of myself.

I state that my name is entered as 'Parthiban' in my Birth Certificate, issued by Kottucherry Commune Panchayat, Kottucherry bearing Registration No. 182, dated 02-08-1994.

I state that my name is entered as 'R Parthiban' in my Aadhaar Card No. XXXX XXXX 9484, issued by Unique Identification Authority of India.

I state that my name is entered as 'Valarmathy' in my Electoral Identity Card, issued by Election Commission of India, bearing No. SJU0044792.

I state that my name is entered as 'Valarmathy' in my Transgender Identity Card, issued by State Government of Puducherry/UT Office of the District Magistrate, bearing No. PO/Kar/2022/50743/1/PO/Kar/ 2022/50743.

I state that my name is entered as 'Valarmathy' in my Transgender Certificate, issued by Karaikal District Collector and District Magistrate, bearing No. PO/Kar/2022/50743/0/PO/Kar/2022/50743-PO/Kar/ 2022/45032.

I state that my name is entered as 'Parthiban.R' in my Transfer Certificate, issued by V.O.C Government Higher Secondary School, Kottucherry, Karaikal - 609 609 bearing Admission No. 2349.

I state that my name is entered as 'Valarmathi' in my Discharge Summary for Gender Dysphonia Diagnosis, issued by Aswini Hospital, Villupuram, dated 23-08-2023.

I state that my name is entered as '' in my Family Ration Card, issued by the Department of Civil Supplies and Consumer Affairs, Karaikal, bearing No. 418509.

C. RAMESH, Commissioner. Hence, I submit that the names described in the abovesaid records viz., 'Parthiban', 'R Parthiban', 'Valarmathy', 'Parthiban. R', 'Valarmathi', '' are referring one and the same person, i.e., myself only.

Further, I state that hereinafter, for all my official and private purposes, I shall be called, known and distinguished only by the name 'Valarmathy'.

The above statements in the foregoing paragraphs are true to the best of my knowledge and belief.

Due to astrological reason, I have changed my minor daughter's name is 'Kothai'. I state that my daughter's name is mentioned as 'Kothai ()' in her Astrological Report, issued by Sri Ayeramkaliyamman Jothida Nilayam, Karaikal-609 602.
