By this deed I, the undersigned Girdhar Gopal Father of Radhika Vaishnav (New Name) previously known as Rashmika (Old Name). Resident of Arjunpura Khalsa, Mangliawas, Peesangan, Ajmer (Raj.) solemnly declare as under :-
1. That for on behalf of myself, my child Radhika Vaishnav (remitter issue) wholly renounce/relinquish and abandon the use of my child's former name of Rashmika and in place thereof I do hereby, assume from this date her name/surname as Radhika Vaishnav so that the remitter issue may hereafter be called, known and distinguished not by my child's former name/surname Ghritee, but by new assumed name/surname of Radhika Vaishnav.
2. That for the purpose of evidencing such my determination I declare that my daughter will at times hereafter in all records, deeds and writings and in all proceedings, dealings and transactions, private as well as upon all occasions whatsoever use and sign the name of Radhika Vaishnav as her name in place and in substitution of her former name/surname.
3. That I expressly authorise and request all persons in general relatives and friends in particular at all times hereafter to designate and address my daughter, by such as- sumed name/surname of Radhika Vaishnav accordingly. In witness whereof I have here unto subscribed and affix my signature on this affidavit to change my child's former name and adopted name on this 2nd day of June, 2025.
