I, Usha Pancholi W/o Shri A.K. Pancholi, age 58 years, R/O Chhawani Road, Navjyoti Press Marg, Kota- 324007 (Raj.), do hereby solemnly affirm and declare as under:-
1. That I am citizen of India. 2. That in various documents my name is mentioned as USHA (old name)ण् 3. That Usha Pancholi and Usha both are my name and same person. 4. That my correct name is Usha Pancholi and I, therefore request to the concerned authorities to may please update and rectify name as Usha Pancholi.
5. That I am enclosing a self attested copy of my Aadhar card and PAN card as a proof of mycurrent name Usha Pancholi.
6. That my above statements are true and correct.
