I, Virapal W/o Jasvindra R/o Ward no. 12, 1 MD, Hanumangarh Rajasthan, do hereby affirm and declare as under;
1. My name is Mentioned as Virapal in my Adhaar card, Votter id card, Jan Adhaar card and other official cocuments and my son or daughter's transfer certificates and 8th marksheet my name mentioned as Sukhpreet.
2. That I want to change my name Sukhpreet to Virapal in my son or daughter's transfer certificates and 8th marksheet.
3. That I am eÛcecuting this declaration to be submitted to the concerned authorities for the change of name.
4. That I will be known as Virapal wife of Jasvindra in the Future. 5. That I have not published in the Gazette for change of name yet.
