I, Mohammed Jakir Husain, son of Mohammed Safi, aged 49 years, residing at Outside Hamalan Bari, Bikaner-334001, Rajasthan do hereby solemnly affirm and declare as under:-
1. That I am filing this Affidavit for changing my name in my son i.e., Sajid Ali's documents i.e., from Jakir Husain to Mohammed Jakir Husain, for the purpose of updating my new name in his Aadhar Card, Jan Aadhar Card, PAN Card, Mool Niwasi, School Transfer Certificate etc., so that my new name i.e., Mohammed Jakir Husainis reflected in his all above mentioned documents.
2. That I am filing this Affidavit for changing my name in my wife i.e., Shabana's documents i.e., from Jakir Hussain to Mohammed Jakir Husain, for the purpose of updating my new name in her Aadhar Card and Passport bearing No- M6242303, so that my new name i.e., Mohammed Jakir Husain is reflected in her all-above- mentioned documents.
3. That I wholly relinquish and abandon the use of my former name i.e., Jakir Husain. I hereby declare that I should be called and known by Mohammed Jakir Husain only in future.
4. That for purpose of evidencing hereinafter in all records, deeds and writings and in all proceedings, dealings and transactions be it private or government, myname i.e., Mohammed Jakir Husain shall be used in place and instead of Jakir Husain.
5. I hereby state that whatever is stated herein above are true to the best of my knowledge.
