1. That I am citizen of India. 2. That in various documents my name is mentioned as Meena Devi (old name). 3. That Meena Devi and Meenakshi Jain both are my name and same person. 4. That my correct name is Meenakshi Jain and I, therefore request to the concerned authorities to may please update and rectify name as Meenakshi Jain.
5. That my above statements are true and correct.
