1- That in my all document i.e. Aadhar Card, Pan Card & etc. my name is showing SHYAM BEHARI DANGAYACH.
2- That my name mentioned SHYAM BIHARI DANGAYACH or SHYAM DANGAYACH in my some other documents.
3- That my actual name is SHYAM BEHARI DANGAYACH instead of SHYAM BIHARI DANGAYACH and SHYAM DANGAYACH.
