AFFIDAVIT
I, Chhouth Mal Gurjar S/o Shri Badri Lal Gurjar resident of Near Shiv Bagichi, C/o Shakshi Kable Network, Bonli distt. Sawai Madhopur (Raj.) do hereby state on oath as under:-
1. That my daughter Aadhar Card has been issued in the name of Lavli Gurjar. 2. That my daughter's other documents (Secondary Marksheet) has been issued in the name of Lucky Katariya.
3. That my daughter's actual and correct name is Lucky Katariya. 4. That now I want to change my Daughter's name as Lucky Katariya in her Aadhar card 5. That Lucky Katariya and Lavli Gurjar is same and one Person. 6. That my daughter's in Aadhar card Lavli Gurjar is to change as Lucky Katariya in place of Lavli Gurjar.
7. That this affidavit submit for Gazette notification for change of name. 8. That it is true statement and I will be responsible.
