AFFIDAVIT
I, Jasbir Singh S/o Shri Ram Singh, age 75 years, R/O 130/6, Shivaji Len, Civil Lines, Station, Kota - 324002 (Raj.), do hereby solemnly affirm and declare as under :-
1- That I am citizen of India. 2- That in various documents my name is mentioned as Jasbir Singh Sahni (old name) 3- That Jasbir Singh and Jasbir Singh Sahni both are my names and same person. 4- That my correct name is Jasbir Singh and I, there fore, request to the concerned authorities to may please update and rectify name as Jasbir Singh
5- That I am enclosing a self attested copy of my Aadhar card and PAN card as a proof of my current name Jasbir Singh
6- That my above statements are true and correct.
