1- My child's old name was "Mayank Singh Rajput". 2- We have changed the name to "Mangal Singh". 3- Both names "Mayank Singh Rajput" and "Mangal Singh" refer to the same person. 4- I request that my child be known as "Mangal Singh" in all future and official records.
