I, MUZAHID HUSSAIN S/o BASHEER AHMED CHISHTI, Age 35 years, Resident of House No. 314, LIGH Scheme First, kamla Nehru Nagar, Jodhpur, Rajasthan do hereby solemnly affirm and declare as under:
1. That I am A permanent resident of the above-mentioned address. 2. That in my previous passport No. T1433279, my name was recorded as "MUJAHID HUSSAIN", which was incorrectly written in English.
3. That in my present passport No. X8872228, I have corrected and changed my name to "MUZAHID HUSSAIN", which is my correct and true name.
4. That I have abandoned my previous name MUJAHID HUSSAIN and adopted the new name MUZAHID HUSSAIN. Henceforth, I shall be known, recognized, addressed and called by the New name MUZAHID HUSSAIN for all purposes.
5. That this affidavit is being submitted to your department for the purpose of publication in the Gazette Notification to recognize the correct name of the deponent as MUZAHID HUSSAIN, as per the prescribed rules for name change.
