I, Prahlad Singh Bhichar, S/o Indra Ram, Aged 50 years, residing at Vpo- Karnpura, Dantaramgarh, Dist-Sikar, Rajasthan, 332023, do hereby solemnly affirm and declare as follows:-
1. That I am the biological Father of my son whose current name is Lokesh.
2. That My son's name has been recorded as Lokesh in official documents.
3. That I wish to change my son's name from LOKESH to LAKSHYA RAJ for personal reason:
4. That henceforth, my son shall be known as Lakshya Raj for all official purposes, and her name shall be updated accordingly in all her records and documents.
5. That this affidavit is being made to officially declare the change of name of my son from Lokesh to Lakshya Raj.
