AFFIDAVIT
I, LAKHVINDER KAUR W/O Kulvinder Singh R/o, Ward No 21, 1 STG Dhani, VPO- Makkasser Tehsil - District Hanumangarh, Rajasthan, 335512 do hereby affirm and declare as under ः-
1. That My Name is Mentioned as LAKHVINDER KAUR in My Adhaar Card, Jan Adhaar Card, and other confedential/Official documents.
2. That my Son NAVJOT SINGH - Daughter JASMEET KOUR Birth certificate My Name Mention SARVJEET KAUR. So my Son NAVJOT SINGH - Daughter JASMEET KOUR Birth Certificate my name to be understood and read as LAKHVINDER KAUR. So I submited a gazzete notification in the affidavit.
3. That state SARVJEET KAUR - LAKHVINDER KAUR is the name of one and the same person and that is my self.
4. That I want to be published a gazette notification to get my name LAKHVINDER KAUR in my Son NAVJOT SINGH - Daughter JASMEET KOUR Birth certificate and other confedential/Official documents.
5. That My Name will be known as LAKHVINDER KAUR in the Future. 6. That I have not published in the Gazette for change of name yet.
VERIFICATION Verified that the consents of the said papers are true and correct to the best of my knowledge and belief and nothing is concealed therein.
